Advance care planning

It is a structured discussion with patients and their families or carers about their wishes and thoughts for the future. Although such discussions may have occurred informally before, it was not occurring with all relevant people or being communicated to others. So the offer of an advance care plan for every appropriate person is now recognised as a key part of good care. (taken from Gold Standards Framework).

 

In Salford, the “Communicate My Care” record is used to record and communicate patients’ wishes and preferences about their future care.  This is an electronic document which records information about a person’s diagnosis, important contacts and details of their wishes, for example around advance decisions to refuse treatment.   It can be accessed by GPs, district nurses, hospital staff and other healthcare professionals.
